Lazaderm Cancellation Patient Policy

Canceling Scheduled Appointments

(Except Liposuction, Microblading, Thread Lift, Y-Lift, and Erbium Re-Surfacing)

Your business is very important to us, and we value your time as well as that of our dedicated and experienced staff.  When you make an appointment with us, this time is carefully planned to meet your needs and reserved in our schedule, especially for you.  We understand that life happens, and your appointments may need to be changed or cancelled but we respectfully ask that you cancel your appointment at least 24-hours/1 business day in advance.  When you miss an appointment with us, we not only lose your business but also the potential business of a patient like you who is hoping for an unexpected opening in our schedule.  By giving us a 24-hour/1 business day cancellation notice, you give us the opportunity to offer your spot to a patient from our extensive waiting list.  If you are unable to give 24 hours/1 business day in advance and/or do not show up for your scheduled appointment, unfortunately, you will be charged a $100 cancellation fee with the credit card placed on file at the time you made your appointment.

Liposuction, Microblading, Thread Lift, Y-Lift, Halo, and Erbium Re-Surfacing Appointments

Liposuction, Microblading, Thread Lift, Y-Lift, and Erbium Re-surfacing are unique when done properly and skillfully require a significant amount of time reserved in our schedule.  For this reason, we continue to require a $500 non-refundable deposit at the time the procedure is scheduled.  The full price of the procedure is expected two weeks prior to the scheduled procedure, which is also non-refundable.  If for any reason the appointment needs to be cancelled, changed, or re-scheduled, there may be a forfeiture of the deposit if less than 7 business days in advance of the procedure.

Credit Card for Appointments

All patients looking to reserve a time for a service with Lazaderm will be asked to place a credit card on file for our late cancellation policy, including when the patient intends to use a pre-paid package or gift card for the scheduled service.  Please have your credit card ready when booking.  Please remember, our goal is not to charge your card but to serve as a reminder to cancel your appointment within the 24 hours/1 business day if you are unable to make it.  We understand that in today’s environment, patients may be hesitant to provide their credit card number to Lazaderm.  We want to assure you that once your credit card number is entered in our system, it becomes encrypted and cannot be seen by anyone.

When you schedule your appointment with us, you are agreeing to this policy.   Please sign up to receive our emails and texts to help you remember your appointments.  For your convenience, we have an automated appointment reminder that goes out both 24 and 72 hours in advance of your scheduled appointment.  If for some reason you do not receive our messages, please understand that it is your responsibility to remember your appointment dates and times to avoid missed appointments as well as the resulting Cancellation Fees.

We apologize for any inconvenience this may cause, however, with the increased number of patients not prioritizing appointments, we regretfully must take this step.  Thank you in advance for your understanding.

Lazaderm Patient Late Arrival Policy

Late Arrival

(Except Liposuction, Microblading, Thread Lift, Y-Lift, Halo, and Erbium Re-Surfacing)

For patients arriving 10 minutes late or more, we may need to ask you to reschedule your appointment as it may negatively impact other patients’ scheduled times.  If we are unable to provide your scheduled procedure due to your late arrival, you could be subject to our Late Arrival Fee of $100.
